The
relationship between thermographic and
musculoskeletal ultrasound findings in the “painful knee syndrome “ Gabrhel J, Popracovб

Z SUMMARY The authors describe various
thermographic and musculoskeletal
ultrasound images of
the painful knee
syndrome, using the images for differential diagnosis of various types of injuries and diseases of
the knee such as socalled “growth pain”, rheumatoid and
other types of arthritis, aseptic necrosis, hydrops or haemarthrosis of the knee joint
due to distorsion, distension or ligament rupture, bursopathy, cysts,
enthesiopathy or sympathetically maintained pain. The combination of both methods seems
to be most accurate in assisting diagnosis. Musculoskeletal ultrasound
images inform about
the structural changes
of a painful knee. Thermographic images provide
information on the pathophysiology of the pain by recording the temperature distribution on the knee surface. The level of temperature may be related to inflammatory reactions or to the activity of efferent nociceptive sympathetic nerve fibers. Information obtained
via thermographic and musculoskeletal ultrasound examinations are important in making decisions on adequate treatment and rehabilitation. KEYWORDS: Knee,

des schmerzhaften Knies verschiedene thermographische und muskuloskeletale Ultraschallbilder, wobei diese Bilder
zur Differentialdiagnose unterschiedlicher Verletzungen und Erkrankungen des Kniegelenks wie sogenannte “Wachstumsschmerzen”, rheumatoide und andere Arthritisformen, aseptische
Nekrose, seröser und blutige Erguss
des Kniegelenks auf Grund von Prellung, Zerrung
oder ligamentäre Ruptur, Bursopathien, Zysten, Insertionstendopathien oder sympathisch unterhaltender Schmerz
genützt werden. Die Kombination beider Methoden scheint eine richtige Diagnose
am besten zu stützen. Muskuloskeletale
Ultraschallbilder zeigen strukturelle Veränderungen des schmerzhaften Kniegelenks. Wärmebilder liefern Informationen über dem Pathomechanismus des Schmerzes, in
dem die Temperaturverteilung an der Oberfläche des Knies bildhaft aufgezeichnet wird. Die Höhe der Temperatur kann mit Entzündungsreaktionen oder mit der durch Schmerz ausgelösten Aktivität efferenter sympathischer Nervenfasern
in Zusammenhang gebracht werden. Die Information aus der thermographischen und der muskuloskeletalen Ultraschall-Untersuchung ist für die Entscheidungsfindung einer adaequaten Therapie und Rehabilitation von großer Bedeutung.
